Teenagers should be prepared for transition to an adult CF service. The choices of adult centre for teenagers in the Oxford paediatric network are generally between the adult CF unit at the Churchill hospital, the adult CF Unit in Leicester and the adult CF unit in Frimley Park. Discussions about transition to the adult service should start at around 14 years of age - including if possible an early decision about which adult centre is preferred. It is possible for families to visit the adult centres before making a decision. All teenagers should be seen in a transition clinic on at least 2 occasions before transferring care to the adult unit. For teenagers who are going to transition to the Oxford CF centre, we offer 4 transition clinics per year - 2 held at the Children's Hospital and 2 held at the adult CF unit at the Churchill Hospital - and both jointly attended by the paediatric and adult CF MDTs. As part of the transition process a summary document about the teenager should be prepared - this is available on the local CF database.
